Vendor Hazard Awareness Training

Employees on this property are subject to the requirements of the Mine Safety and Health Act of 1977, as well as any Company Safety procedures. Employees must be made aware of site-specific hazards and any applicable safety procedures or rules. High Visibility vests are required at all times. Passenger vehicles are allowed in the designated parking areas. During your time on our property, observe all rules and regulations, including speed limits, and carefully follow all verbal instructions given by plant management or other authorized personnel. Immediately report to the plant supervisory personnel any unlisted hazard you may encounter. Failure to adhere to these requirements may constitute dismissal of employment, and you may be denied future access into the facilities. Please feel free to discuss any rule, or gain clarification about any matter of safety with the pit supervisor. (30 CFR Part 46 Section 46.11)

Instructions:

Read each statement carefully and check off once understood.

Personal Protective Equipment:

Do not enter restricted areas without wearing all personal safety equipment. If you must exit your vehicle, you must wear a hard hat.

Traffic Control:

Wear your safety belts while traveling over the property. Traffic flow is on the right. Obey posted speed limit and warning signs.
Look and listen for large trucks and heavy equipment. Yield to heavy equipment at crossings – the operator’s visibility is limited.
Listen for back-up alarms and stay clear of the path of moving vehicles.
Do not drive or park within 25 feet of any stockpile or pit wall.
Watch out for and listen to personnel on the grounds.
Passengers are not permitted to ride anywhere on a vehicle not intended for passengers. Never dismount from a moving vehicle.

Equipment and Machinery:

Check in with the supervisor in charge of the area assigned to work, and wear all required personal protection equipment prior to entry.
Obey all Lockout/Tagout/Tryout procedures.
Keep hands and feet away from moving parts. Be certain clothing is tight fitting and shoe strings are tied.
Obey all danger, warning and instructional signs.
Do not step over moving conveyors. You may walk under in designated walking areas only.
Be alert and remain clear of moving equipment. Make sure the operator knows you are there. Be aware of any special traffic or driving hazards. Never Park in front of or behind stationary equipment. Never Park under conveyor systems.
Use designated walkways, catwalks, and handrails.
Use fall hazard protection where necessary.

Your Conduct and Safety:

Obey the instructions of the supervisor attending. Observe “No Smoking” signs and areas where posted.
Walk – don’t run, unless there is imminent danger. Horseplay, fighting and practical jokes are prohibited.
Use or being under the influence of alcohol or a controlled substance is prohibited. Possession of illegal drugs, alcohol, guns or weapons is prohibited within the property.
Use of Cell phones is permitted while driving in the pit. You must pull over to a safe area before initiating a call.
Be familiar with all MSDS’S for all hazardous materials you may contact while on the mine site.

Accidents & Emergencies:

For Emergencies, dial 911.
Report all accidents, emergencies, and near misses to the supervisor or scale house.
In the Case of an Emergency, assemble at the scale house.

The preceding hazard training has been discharged in accordance with our approved training plan. You shall fully indemnify and hold
harmless from any and all liability, loss or expense (including attorney’s fees, expert witness fees and costs) incurred as a result of
your negligence or omissions at this site.
